Diagnosis: Early acute cholecystitis.
Treatment: Inpatient Hospital Admission.
The insurer denied: Inpatient Hospital Admission.
The denial is upheld.

The patient is an adult female with history of gastric sleeve who presented with early acute cholecystitis. The patient underwent laparoscopic cholecystectomy. She was discharged the same day after her surgery. Her white blood cell (WBC) was normal, and her liver functions tests (LFTs) were normal. The operating report showed edematous gallbladder.

The proposed inpatient hospital admission was not medically necessary.

The patient underwent inpatient admission for uncomplicated acute cholecystitis. Uncomplicated acute cholecystitis is generally treated at a lower level of care unless there are contributing factors such as complications or comorbidities that warrant inpatient admission. This patient had no issues after the surgery or medical issues prior that necessitated further inpatient monitoring. The patient had a white blood cell (WBC) count of four and only had evidence of early acute cholecystitis. The patient was discharged without complications. There was no indication for inpatient admission.
